ADMISSION NOTICE NO- 81/2008
SESSION JANUARY 1ST 2009

Applications have been called for PGI CHADIGARH {PG/DM} entrance exams. These are the following details

 

• Date of MD/MS entrance exams: 13-12-2008 (Saturday)
   

• Date for DM/Mch/MHA entrance exams: 18-12-2008 (Thursday)
  
   

APPLICATION FORMS:

Application forms can be obtained in 3 ways:

 

1. Download online application forms at http://pgimer.nic.in . The form should be duly filled & sent to the address given below along with a demand draft in the name of the DIRECTOR,PGI. (Rs.500/- for GEN/OBC/OPH & Rs.400/- for SC/ST).
    

2. Application along with prospectus is available from the office of the REGISTRAR PGI personally on a payment of Rs.500/- for GEN/OBC/OPH & Rs.400/- for SC/ST.
    

3. Applications can also be procured by POST for which a request must be accompanied with a self addressed thick envelope measuring 24X15 Cm's and bearing postage stamp of Rs.30/-.& a DD of Rs.500/- for GEN/OBC/OPH & Rs.400/- for SC/ST. DD/s should be preferably taken from SBI branches & should be made in favour of THE DIRECTOR; PGI;Chandigarh. DD,s should be payable at SBI medical institute branch (code-1524).
    

4. Prospectus for DM/M.ch will be available only for Rs.500/- irrespective of the caste as there is no reservation for the same.
    

PLEASE NOTE:

• Last date of receipt of applications: 27-10-2008 (4.00 PM).
   

• It is in the interest of the candidate to send the application by SPEED POST.
   

• No request for the supply of forms will be entertained after 22-10-2008.
   

• Fees once paid is not refundable.
